Person Centered Planning

Person Centered Planning facilitates the development of life plans that focus on a person’s unique interests, strengths, gifts and resources as the foundation for all aspects of planning.

OTAC has demonstrated a long-standing dedication to the values and practices of person centered planning. At the heart of OTAC lies a deep and abiding commitment to expanding the meaningful application of person centered planning principles in the lives of people as they are supported by their families, friends and through more formal, systematized services.

OTAC’s person centered planning staff are very skilled and experienced in a variety of internationally recognized planning processes. We provide levels of technical assistance and support ranging from facilitating individual plans through mentoring others to become skilled trainers in some of these planning processes. OTAC staff has demonstrated skills in the following planning processes:

Essential Lifestyle Plans (ELP) support people to get better lives. These comprehensive plans detail what is important to the person as well as providing invaluable information about how to deliver supports. This helps the person to live the kind of life THEY want while also attending to important issues of health and safety.

Project staff has wide-ranging experiences in applying Essential Lifestyle Planning with people supported both by family members and through professional services. They have helped bring ELP into supports of people who have developmental/intellectual disabilities, severe and persistent mental health issues, physical support needs and with seniors in aging services. OTAC can develop individual plans as well as to teach and mentor others to become skilled facilitators and trainers in ELP. OTAC maintains an active, ongoing relationship with Michael Smull, co-developer of ELP, assuring OTAC staff is at the leading edge of this process.

Making Action Plans (MAPS) ask eight identified questions that are designed to create a holistic view of a person by listening to and collecting information about their personal story. From the focused sharing by the person and by their gathered supporters, the group charts an action plan which honors the person’s gifts and capacities as well as outlining specific actions and commitments designed to assist the person reach their stated dream.

Personal Futures Planning is a graphically based planning process designed to describe a desirable future for a person and then decide what activities and supports are required to move towards that future. This process brings the focus person together with people who have held key roles in their past as well as in the present to gather information and to collectively develop and commit to actions leading to a meaningful future.

Planning Alternative Tomorrows with Hope (PATH) is a tool to help move a group of individuals to develop a positive AND possible future. The “pathfinder” of focus can be individuals, families, organizations, schools, or other service or business groups. The pathfinder and other committed supporters begin to dream together and develop a clear vision for the future that can be reached within an identified timeframe. Through a guided process and graphic facilitation, the group then fashions the supports and resources that they believe are needed to reach that goal. At its conclusion, the group has a clear, detailed vision and concrete steps and timelines in place to move forward.
